How to irrigate the ear –

  1. Grasp the OtoClear tip and twist onto the luer lock fitting of the adapter wand until it stops.
  2. Fill the water reservoir of the delivery device with warm water (approximate body temperature). DO NOT use cold or hot water as it may cause dizziness in some patients.
  3. Prime the unit by discharging into a sink.
  4. Insert O to Clear tip fully into the ear canal. The flared design of the O to Clear tip protects against over-insertion. Tip: Fully inserting O to Clear Tip will help eliminate backsplash.
  5. Place a basin beneath the outer ear to capture draining effluent. Note: The O to Clear tip directs the water towards the walls of the ear canal creating a turbulent lavage that effectively dislodges the cerumen. The exit portals control backsplash and deliver the effluent into the basin.
  6. Reexamine the ear canal. Repeat the irrigation procedure as needed. Sometimes a large plug of wax will not exit through the portal. In these cases, we recommend the use of a Safe Ear Curette™ to extract any remaining cerumen.
  7. Remove any remaining water in the ear canal with an absorbent sponge. When procedure is complete, remove and dispose of the single-use OtoClear tip.

Is the OtoClear Tip suitable for all patients?

The OtoClear Tip can be used on patients of all ages. However, in general, ear irrigation including OtoClear Tip, is contraindicated for patients whose eardrum may be perforated, where tympanostomy tubes are present or suspected, or active infection is present or suspected.

Is There a CPT Code for all patients?

A new CPT code, 69209, provides a specific billing code for removal of impacted cerumen using irrigation/lavage. Like CPT 69210, (removal of impacted cerumen requiring instrumentation, unilateral) 69209 requires that a physician or qualified healthcare professional make the decision to irrigate/lavage. However, unlike 69210, 69209 allows removal to be carried out by clinical staff.

Criteria for both CPT code 69209 and 69210 are the same and documentation to support the medical necessity of service is necessary to bill a separately identifiable service to an E/M. Each are considered unilateral and require modifier 50 to establish a bilateral procedure.

Because CPT code 69209 is new, many payers have yet to determine its status. Please consult a billing specialist prior to using.
